PUNXSUTAWNEY - After an extensive joint investigation with the Jefferson County District Attorney's Office and Punxsutawney Borough police, Cindy L. O'Neill, 31, of Irvona and Scott Cary, 42, were determined to be the overseeing adults for the care of a 3-year-old child that was left unattended and walked/fell into the Mahoning Creek behind a residence on Lane Avenue in Punxsutawney.
The child died as a result of drowning and the body was recovered a short time later downstream.
According to the state police at Punxsutawney, the incident occurred May 21, 2011, within Punxsutawney Borough, Jefferson County.
